Application Materials

To apply for the Upward Bound program, students must complete an application and provide a copy of their recent grades and school transcripts.

Application Information Required:

  • Student information
  • Student essay
  • Parent/guardian information
  • Self-reporting income
  • Permission to access records - completed by student and parent/guardian
  • Student reference form (2) - completed by teacher and/or counselor

OR Give completed application packets to Upward Bound staff located at East, West and Expo High Schools
OR Give completed application packets to the main office (East and Expo) or at the counselor's services office at West High School
